If you have an outstanding invoice for either event, please email the following documents to 5starandmcc_inquires@mdstad.com:
- Unpaid Invoice
- Executed Contract
- Proof of Work/Completion
- Current W-9
MSA is only collecting information at this time. This is not a guarantee payment eligibility. We will reach out directly if additional documentation is required.
